<font color=purple>Make-up Pictue Day • ID Photo • Monday • October 5th (IF YOU PURCHASE A PACKET, YOU GET FREE DRESS. SEE BELOW FOR POLICY/RULES)

Here are the steps to pay for your child's school pictures:

1. Return the Picture-Day envelope with money or a check on Wednesday, September 30th, with your child.


2. Visit (copy and paste in your browser)

• Enter Picture Day ID (DC015580Y0) for Picture Day.
• Order and print the receipt to give to the photographer on Picture Day.

Pay online with: Visa, Master Card, Discover, American Express

Pictures will be taken during your child's physical education class.

Students purchasing picture packets may have free dress. To qualify for free dress on picture day, students will need their picture envelope with money or a check, or proof that the picture packet was purchased online.

Free-Dress Policy/Rules for Picture Day:

1. No tank tops, or shirts that expose the midriff
2. No hats (unless preapproved for medical reasons)
3. No slippers or backless shoes of any type
4. No shorts with knee-high socks
5. No clothing with Raiders, Yankees, Kings, or other sports logos
6. No items of clothing, which depict any form of profanity, gang identification, violence, drugs, alcohol, tobacco, vandalism, weapons, a sexual act, or is offensive to any gender.
7. No metal-studded leather bands or collars.
8. No exposed sharp objects of any kind may worn as jewelry on clothing, or on backpacks, (e.g. straight pins, safety pins.)

This form provides the school with the required contact information for your child. It must be completed at the beginning of each school year (or whenever there is a change in information) and provided to the school.

It is critical to the instructional budget of Van Nuys Middle School that all families complete a meal application. VNMS lost close to $200,000 in the 2013-14 school year because many families did not complete a meal application.

Tdap Booster for 7th-Grade Students (Click Here For Immunization Clinics)

<font color=blue>Tdap Booster for 7th-Grade Students (Click Here For Immunization Clinics) State law requires all students entering or advancing to the 7th grade to provide proof of a Tdap booster before the start of the school year. Students who do not have proof of Tdap immunization on file at school will not be allowed to attend school.

It is strongly recommended that 6th-grade students bring proof of their Tdap immunization to the school now to avoid exclusion next school year.
